In order to be considered for a Terry Fox Humanitarian Award,
students must submit one application form in Microsoft
Word format, three referee forms and one official transcript of
the past two years of schooling - no other forms are required.
Incomplete application forms will not be considered.

Deadline
is February 1st of every year.
You may type your answers directly on to the Microsoft Word document
OR print your answers onto a blank sheet of paper and attach them
to the application form OR re-set the margins on your word-processor
to print directly into the spaces provided. Application forms
filled out by hand will NOT be accepted, however for the first
two pages you may hand-print your answers in bold letters. Application
forms must be mailed in, and e-mailed application packages will
be discarded.
Once you have your completed application form, all three referee
forms and an official transcript of the past two years of schooling,
please mail
the package to the Terry Fox Humanitarian Award Program, 8888
University Drive, AQ5003, Burnaby BC V5A 1S6.
Notes:
- If
you would like confirmation that your application form and all
relevant documents have been received in good order, please
include a stamped, self-addressed envelope with your application
package.
- Application
forms must be printed out and mailed along with supporting documents
to the Terry Fox Humanitarian Award Program offices. They cannot
be sent in electronically (ie. faxed or e-mailed).
Application forms and accompanying documents must be post-marked
by February 1st of each year. Forms received after
that date will not be considered, nor will they be held over for
the next year. Application material will NOT be returned to students.
All
applicants will be informed of the status of their application by
the end of June of each year. Please inform the
Terry Fox Humanitarian Award Program office of any changes in your
contact information. |
